db2f9be009 sa2: add some logic to implement Full Speed. 
Because we run a frame at a time, this is a bit different than in AppleWin.
Run up to 5 extra frames.

f144cb037f SDL: Support Mouse. 
In ImGui there are a couple of issues:
1) it ignores the menu area when computing relative motion
2) does not work well with windowed video
